Request a Sick Note for Work

You do not require a doctor’s sick note (also known as a fit note or MED3 form) for any illness lasting seven days or less. Instead, you can complete a self-certification form (SC2) which you can request via your employer or our reception team.
A fit note is required if you have been off work for more than seven days due to illness. You can request a fit note in the following ways:
- Online: Use our eConsult to request a repeat sick note.
- By Phone: If you are unable to use eConsult, please call the surgery to make a request.
Who Can Issue a Fit Note?
Fit notes can be issued by a GP, hospital doctor, nurse, occupational therapist, pharmacist, or physiotherapist. If you are receiving ongoing care from a specialist, they may also provide a fit note.
What If My Fit Note Expires?
If you need an extension to your sick note, you will need to request a repeat. This may require a follow-up consultation, depending on your condition.
Can I Return to Work Before My Fit Note Ends?
Yes. You do not need a separate note to confirm you are fit to work. You can return to work as soon as you feel able, even if your fit note has not yet expired.
