Housing Application Letters

We do NOT provide GP letters for housing applications.

If you are applying for council or housing association accommodation on health grounds:

  • You do not need a letter from your GP. Complete the self-assessment form provided by your local housing department — this does not require GP input.
  • If additional medical information is required, the council or housing association will contact us directly with a specific request. You will need to provide written consent for us to release any information.
  • We can only provide factual information from your medical records, such as diagnoses, severity of illness, and current medication.
